Raziel Health and Baptist Health South Florida Announce Strategic Alliance to Deliver Remote Continuous Care

In order to expand the availability of additional cardiac services, Baptist Health South Florida and Raziel Health today announced a strategic partnership to provide Baptist Health South Florida patients who qualify with heart failure and/or complex hypertension with access to Raziel’s unique continuous care platform and remote patient monitoring services.

With the use of Raziel’s proprietary platform, doctors and care teams may provide ongoing medical care to patients who are chronically ill, have complex medical conditions, or are transferring from the hospital to their homes. For patients with chronic health issues, especially elderly, efficient remote monitoring and diagnostics is quickly emerging as the new standard of treatment. Through a combination of FDA-approved technology, individualised nursing care accessed from remote places, and algorithms created by top clinicians that proactively identify dangers and support suggested treatment regimens, Raziel offers a better approach.
