Remote patient monitoring (RPM) is the remote collection and analysis of patient physiologic data used to develop and manage a treatment plan related to a chronic and/or acute health illness or condition. This involves a patient who takes a biometric reading at home from a device, such as a blood pressure cuff, glucometer, or scale. The device, prescribed and obtained at a low cost, enables patients to have an enhanced view of their health.
