In order to expand the availability of additional cardiac services, Baptist Health South Florida and Raziel Health today announced a strategic partnership to provide Baptist Health South Florida patients who qualify with heart failure and/or complex hypertension with access to Raziel’s unique continuous care platform and remote patient monitoring services.

With the use of Raziel’s proprietary platform, doctors and care teams may provide ongoing medical care to patients who are chronically ill, have complex medical conditions, or are transferring from the hospital to their homes. For patients with chronic health issues, especially elderly, efficient remote monitoring and diagnostics is quickly emerging as the new standard of treatment. Through a combination of FDA-approved technology, individualised nursing care accessed from remote places, and algorithms created by top clinicians that proactively identify dangers and support suggested treatment regimens, Raziel offers a better approach.

“Raziel is part of Baptist Health South Florida’s long-term strategic vision to provide differentiated patient engagement and population health for the greater South Florida market. This innovative technology allows for care beyond the traditional clinical setting and identifies possible health issues for early intervention. We are partnering with Raziel for its unique integration with Cerner and track record of clinical excellence”, “Raziel’s continuous care will be available to patients identified and managed through Baptist Health’s Population Service organization, Disease Management Department in partnership with its physicians.”

Jonathan Fialkow, Chief Population Health Officer at Baptist Health South Florida, and Deputy Director of Miami Cardiovascular Institute

“We are extremely excited to work with a premier, market-leading organization like Baptist Health South Florida. Our shared vision is for the program to become, over time, one of the largest active remote monitoring programs in the country.”

Jeffery Gruen, MD, Founder and CEO of Raziel Health

Key features of the Raziel platform include:

  • Based on a three-year academic center/government study on large populations, significant drops in emergency room visits (by 60%), hospital admissions (by about 40%), and 911 calls (by 26%).
  • Within 60 days, physiological measurements like blood pressure and serum glucose typically experience rapid, clinically meaningful drops.
  • Outstanding patient satisfaction, with 94% of users continuing to use the platform after three months (3x the CMS-published industry average)
  • increased vigour and lifespan of the patient

Beginning immediately, clinicians and patients at Baptist Health South Florida will have access to Raziel’s ongoing treatment.
